you don't need to use afs program for teams.
just copy img files to C:\Program Files\KONAMI\Pro Evolution Soccer 2011 DEMO\img

and copy pes-hio.exe and setting.txt to C:\Program Files\KONAMI\Pro Evolution Soccer 2011 DEMO

then open pes-hio.exe choose your teams and it will start automatically

The patch also does not work for me. The menu may look like it was patched, but I only Bayern and Barcelona to choose from.

Where are Real Madrid and Inter etc?

I installed your patch and it's still only Barca + Bayern Munich.
